Sounds like everything worked as intended, as seat belts are usually not needed on coasters. I've personally never ridden Mamba, but I have ridden Steel Force, which is mostly floater IIRC. Does Mamba have more ejector?

Bad reporting and bad statement from the park. "Implemented modifications requested by the Fire Marshal" makes it sound like the ride wasn't up to standards when this incident occurred.

This is why ride hosts pull on the seatbelt. What are the chances that both a rider’s seatbelt isn’t fully clicked into place and the lap bar or shoulder harness malfunctions at the same time. One in a trillion?

The video segment does seem to heavily imply that if the couple wasn’t there to save the day, the girl would have flown out?

Is that true? I’m guessing the lap bar would have been enough? But depends on how the girl is built??

That's probably what the people believed. If local news actually did some journalism they should have spoken to a safety expert who would say that this was a non-event. Sensationalism at its worst.

If local news actually did journalism this country would look radically different.

This article does feature quotes from Missouri’s Department of Public Safety. Apparently the ride failed inspection because "more than 20 belts on the Mamba did not lock as required."

If I remember correctly (been 2 years) I don’t think Mamba has the airplane buckles (ie the ones you have to pull the front 180 degrees to unbuckle) but the push button ones. It’s the end of the season and that buckle relies on a spring, if you were trying to defer maintenance I could see letting a fatigued buckle not be replaced.

OK, that's journalism. If that's solely on the maintenance guy for the ride, he needs to be fired. If that's because the park or SF is cutting back on maintenance budget, then I'm very concerned.
