Sounds like everything worked as intended, as seat belts are usually not needed on coasters. I've personally never ridden Mamba, but I have ridden Steel Force, which is mostly floater IIRC. Does Mamba have more ejector?

Bad reporting and bad statement from the park. "Implemented modifications requested by the Fire Marshal" makes it sound like the ride wasn't up to standards when this incident occurred.

This is why ride hosts pull on the seatbelt. What are the chances that both a rider’s seatbelt isn’t fully clicked into place and the lap bar or shoulder harness malfunctions at the same time. One in a trillion?
