I don't know who else can claim to this, but on Anaconda's second loop at PKD, I "greyed-out" once and had a dizzy feeling afterwards for about 30 minutes. It seemed like an overall confusion. Today I went and my vision got blurry also, on the same part.
Believe it or not, I actually blacked out at the top of the loop on Deja Vu going backwards, and zoned back in during the first part of the cobra roll. That may have been because we were stuck at the bottom of the second tower for 20 minutes, but still. Even now I still can smell blood in my nose throught the course of the ride, but that doesn't stop me of course!
Goliath makes me gray out in the end of the helix if I'm in the back rows, otherwise I get a little bit of tunnel vision. I love it.

When you’re relaxed, there is nothing to keep your blood from rushing down, leaving your brain with less than normal oxygen levels. [Your brain is very sensitive to drops in oxygen] Your vision fades when less oxygen is available. Dehydration [about 90% of Americans are] means that you have less blood volume to pump to your brain.
Think you’re your body and blood like a supply/demand relationship. The potential space inside your body is the demand; your blood volume is the supply. The closer supply gets to meet the demand, the less sensitive to G’s you are. There are several things you can do. You can be properly hydrated, that will increase the supply of blood. Lowering the demand is easy, squeeze every blood vessel you can think of. As mentioned before, taking a deep breath and holding it hard. But you also can contract every muscle in your body. This will squeeze your blood vessels together making you less vulnerable to G forces. The only way you can better prepare yourself, is training and pressure suits. That’s not exactly for everyone. :)
I heard someone say in line for Goliath that if you do "stomach crunches"(tighten your stomach muscles) right before the helix it can help you not blackout. Also, drinking a Coke or Pepsi with caffeine might help. It seems that if I go on a ride with high g-forces later in the day when it is cooler and the sun isn't beating down, that's better for me. (If you go on a ride when it is dark outside, you might not even notice if you gray-out :)
In my opinion, it seems that toward the front of a coaster train, the g-forces are higher when entering an element and then you have to go continue going through the inversion already affected which might be worse than toward the rear of a train. I hope that makes sense.
I almost passed out on the second loop of Shockwave at Six FlagsOver Texas. MF at Cedar Point also made me gray-out before the first tunnel.
The only grey out that I've ever gotten on a coaster (or any thrill ride for that matter) was on Millennium Force on that overbanked turn. I lose all peripheral vision, get that fuzzy feeling in my head, and get hazy, all three mentioned in the post. It's really a cool feeling, but I don't know how many times I'd be comfortable doing it in a single day.
As for headrushes, that's a completely different story, I'll get those from anything, just getting up gives me a headrush every time, but some of the better headrushes on coasters include the second loop on Shockwave (SFGAm), the first vertical and second vertical loops on Batman at SFGAm, the loop on Revolution at SFMM, and random parts on Raptor at CP. There's other spots that will give me a headrush on coasters, but those are the ones that really stick out to me.

Top Gun had a helix? lol I was almost out after that corckscrew! That has to be the most intense coaster I have ever been on. Well that and Goliath. Some people say Goliath isn't long enough, but I think if it was any longer it would be too extreme. Coasters aren't built like Goliath very often these days!

I have greyed out many times, but only on one coaster, BKF.
I always grey out at the bottom of the drop. Everything just gets hazy and I start to get tunnle vision. But I also greyed out in the final helix once or twice. I think greying out just adds to the ride!
