I can't be certain who posted it 'cause this client formats the page funny, but wrt. the Looping Star blocking: Loose wood blocks is the correct way to do it. On the other hand, if you see a ride sitting on cinder block, I suggest you run like hell.
Funny thing is, I've seen some clearly unsafe stuff, and most of the stuff described in this thread so far, while "improper" or simply "wrong" is not necessarily unsafe. Not physically checking lap bars, for instance, isn't necessarily unsafe; merely incorrect. Besides which, do you not always check your own anyway??
You want unsafe? Let's talk about the operator who decided to start a Giant Gondola Ferris Wheel rotating while passengers were exiting the ride, for instance. Or the operator who waited until the train was entering the station before deciding to leap across the track. Or the operator who decided to cross in front of a Sky Ride gondola that was sitting, ready to go, in the automatic tripper. Or the entirely untextured concrete one park used on its midway that is slicker than ceramic tile or diamond plate when it is wet.....
