Has a coaster ever derailed?

What happened on Lightnin' Loops at SFGAdv? I've heard the story somewhere vaguely, but I can't remember it...

Magnum Broke an axel in 1995 on car two of (I think) the black train. Nobody was hurt, but some were bruised. The car, still ripped all of the brakes off as the fin passed over them at an angle. What a terrible sound it made!
I don't know of any case of a modern coaster simply flying off the track, unless you count the special effects in the movie "Rollercoaster". A few clarifications on the incidents mentioned above.

West Edmonton Mall: The train did not actually derail. All of the wheels stayed on the track. What happened was the car body separated from the lower chassis and wheels which remained attached to the track. The carbodies still partially attached to the chassis struck a column in the mall resulting in the fatalities.

King Cobra at PKI: A wheel assembly failed resulting in the violent shaking of the passengers with a couple injured pretty badly. Other than the failed wheel assembly, the train stayed on the track. I had planned on riding it the next day.

I remember when it happened reading an article on it. I put the article somewhere, but I cant find it now. But I do remember it happening.

At SFOT in the 70's the " Big Bend" was in its helix and derailed, and was taken out. It was located were splash water is located. 9 people were killed. My mom was there the day it happend, she said.

That was a major STUPID mistake on a ride operator's part. The (19 yr old) girl got into the lightnin' loop's seat (next to her boyfriend). The harness was down, but she stepped in (like the last trip was with a single rider and they didn't pull up both harnesses when they got out). Before she had a chance to pull her harness down (or I should say before she had a chance to put it up, sit down and pull it down) they launched the train out. For anyone who's been on one of those archaic (but scary!) Arrow shuttle loops, the drop off has pretty intense negative G's. She flew out and got wedged between cars, then fell to the ground (after being run over). It was a truly horrible MISHAP. I remember one of the articles said she was screaming for her boyfriend.

I wonder what happened with the ride operator. Maybe they're in jail??? I couldn't live with myself if I did something like that. Imagine, goofing around with your crew, smaking gum, then all the screams and someone's dead. Ug.

Demon at SFGAm had something go wrong with one of the wheels or something and got stuck upside down, I forget what actully happened.

Here we go again... The Big Bend NEVER killed ANYONE! It may have derailed but if it did it didn't kill anyone, and it was NOT the reason why it was removed. The death on the Roaring Rapids in 1999 was the only guest fatality at the park in it's history, and the coaster was later removed due to maintenance problems and being too close to the entrance. I suggest getting your facts straight if you want a reputable reputation on Coasterbuzz.
