Does anyone know how this could have happened is it the mfg or the operator, and is it related to the water level?

Honestly, first thing that comes to mind is rider behavior (not sitting, and then coming down hard while the ride is in motion). Not knowing any details about any supposed incident, though, it's impossible to say anything.
