Briar Crutchfield, a spokeswoman with Alberta Occupational Health and Safety says a 47-year-old worker was trying to remove a patron’s hat that was lodged in a track at the Galaxy Orbiter shortly after 12:20 p.m. That’s when the worker was stuck by a rollercoaster that was loaded with passengers as it was descending around a spiral.

I have never, ever owned a hat that was worth getting killed over. Even my hat with Derek Jeter's signature isn't particularly important to me.
I can't believe that they don't tell workers up front that entering the ride area without locking it out is grounds for termination. If that isn't policy, it should be.

What's up with workers being hit by spinning coasters!? This is the third one I can think of - but this person lived.
Lock out, Tag out is the law.
No excuse for this one unless its proven they were never trained that way.

I think the (relatively) low speed of the cars tends to make people complacent.
Dave's signature should be posted directly adjacent to the time-clock for all employees who work on or around rides...machines simply do not care.
