For once I agree with Jeff 100%. He may have had a heart attack, a stroke or even an anurism but his body would have come back in one peace if only the lapbar had been in proper possition.
And why wasn't it in proper possition? It wasn't checked and there were no other messures to tell that the bar was down far enough.
