You can find some info on it on the web.  Just do a search about the mindbender accident, and maybe throw in a search word like Edmonton.  That should help.

It was an accident on the Mindbender at Edmonton mall, not long after the coaster was opened.  One of the cars wheel assemblies deteriorated and the car came off the track during a loop and sent passengers to the ground.  Death ensued.  I don't have the exact facts, but that is what I recall about the tragedy.  You will have to research it to find out the 'facts.'

The Mindbender is a hyper-intense (so I am told) Schwarzkpof multi-looping coaster at the West Edmonton Mall in Alberta. See photos and learn more here.

In essence, a wheel carrier sheared away from the back end of the last car. This allowed the car to derail and slam sideways into a support pillar. The lap bar mechanisms in that car were tripped and riders were thrown. Three people were killed. Since the incident, the cars have been rebuilt as two-axle cars, shoulder bars have been added, and the ride still operates.
