I know I'm missing a few (possibly more) but that should get you started.
Indiana Beach, Knoebel's, PKD, Holiday World, and Fun Spot all have scooters. Someone else can probably answer the rest of your questions.

Good ones I've ridden: Fun Spot, PKI/PCar, and KG. Don't be TOO surprised if IB decides to allow snapping in the next year or two...they were *definitely* snap-able....;)
Hershey, I had no luck with, but some of the better *pilots* were able to get in a few good snaps (the op seemed more amused than annoyed, thankfully).
Who makes them? In the "good old days", Bisch-Rocco. Now, Larson.
edit: I don't know who *Scooter* is (other than perhaps Phil Rizzuto), but I could use a fling myself...;)

You still have Zoidberg.... You ALL have Zoidberg! (V) (;,,;) (V)
We rode IB's Air Crow on one of the first days it ran and it definitely had the potential to be a "snapable" ride. Time will tell if IB lets riders do that. But if any park would, it would be IB!
conneaut's are good... the snap fairly well...

And for the record, we do not allow snapping. (Although I've been told it's a moot point, as the tubs are too heavy anyway.)
Thanks, Paula (rode 'em once; felt sick the rest of the day...but I never did "fling"!)
