The Evergreen State Fair roller coaster ride known as the Zillerator had a car containing four people jump the rail and come to a halt at the top of the ride, about 30 to 35 feet up. No one was hurt, and safety systems worked as they should, but the fire department had to use a ladder to get the guests out.

Congo Falls - 90 (one day)
The exact same thing happened to me at a carnival in the late 80's. On a similar coaster, we slammed to a stop. The ride op (get this) climbed the coaster and hammered whatever it was he had to hammer and rode down with us almost on my shoulders. It was pretty unnerving, but to him seemed like a common everyday routine.
We were all seated straddling each other, and our knees got slammed pretty good into the bar holding us in.
At the bottom..no complimentary Elephant Ears, no free ride on the Zipper, no "sorry you wrecked"...nothing. . .but a swollen knee. . .
We hobbled home.
