Has anyone ever puked on a ride or from a ride? I haven't, but thought I was going to when I rode Superman about 20 times in a row.
Repeated rides on Raptor, at night, after eating (that's a lot of conditions) makes me uneasy, but that's the worst it has ever been. Ditto for Alpengeist after having a few, uh, "beverages" from the parent company.

I never puked on a ride but this year I saw a few clean ups on, Loch Ness Monsters, Serial Thriller, Apollo's Chariot, and on the Steel Phantom to name a few of the top of my head. Oh then there was a girl passing out and puking in line for S:ROS at SFA I think she was on somthing though.

never on a coaster, but there was this one time (at band camp...sorry nevermind) anyway, when I was in day camp (age 11) I went to santas Village in IL and we got on a snowball themed 'teacup' type ride. At that time I didnt know NOT to look up on these things. None of us actually earled, but we all fell a couple of time getting off. I though I was going to, but ended up holding it in.
The only time I actually saw it, was two years ago at the Champaign (IL) county fair. We were waiting in line for the zipper when all of a sudden they had to pull out the hose. Needless to say, we bypassed.

I've never puked on a ride but twice this summer, someone puked on the Mantis while I was waiting in line to ride. Of course, the ride ops had to do some cleaning and then they ran that particular train empty for ten minutes.
Mine is similar to 2Hostyl's, those tea cups can be killer.
I rarely feel sick on rides, even most spin and pukes, but the Coaster Zombies' ride invasions at SFA almost did me in. All of us coaster lovers boarded the tea cups at the same time....and we went insane. I don't remember who was spinning my particular cup(in fact I barely remembered where I lived afterwards), but we were going around at some seriously high RPMs. After I got my footing, I was OK, but I seriously came close to vomiting at one point.

And for all of you who dont know, never do like me and WildOne (and I'm sure SFAGuy too) and try and pile Five adults in one of those things...I had a headache for the next two days after the C-Z event. So many spin&spews...so little time. I'm just glad we didnt do the Iron Eagle (roto-shake). I would have been done for!
I personally never have but I have a friend who did this year on the Magnum.
Let me start by saying the first thing that led to this was a 3 hour drive and about 5 beers . The second thing that lead to this happened on the Mantis. I think you guys will know what I mean when I say his family jewels got rocked. Third, he ate a full course meal at Coasters. About an an hour later as we were leaving the park for the day, we got in line for Magnum and he followed ("got to ride it night.") He made it the whole way through the ride and we're stopped before entering the station (mind you he was sitting with another friend directly behind us) and I hear "I shouldn't have ate" and he proceeded to turn his head and puke. I hear the retching and immediately plug my ears because I know if I listen to this, I'm going to join him. He was able to stop long enough to pull into the station, unload and head straight to the railing only to begin puking again. Of course, he will never live this down. He bounced back really well however. On our walk back to the hotel he stopped and bought a quart of ice cream and ate it all.

Nothing beats the Ohio State Fair last year......if you are familiar with the High Roller, you'll understand! We saw a girl get off and her face said it all. Sure enough! It is kind of an oval Ring of Fire, only it is hinged at each end and rotates sideways while the car traverses the oval track.
I believe I blew chunks when I was little after a corn dog and a spider ride, but nothing in the last 2 decades!

I've never had the displeasure of doing this but we saw a guy hurl on Whizzer at SFGAm - before it even left the station! Do you think it could have been something he ate????

Never tossed cookies on a coaster, but plenty of flat rides.
Scat, Octopus, Propeller Spin, Hammerhead (also called Salt & Pepper shakers) Tilt-a-whirl, Teacups, Zipper, Swinger, Dragon Swing, and most likely a few more that I can't recall. All of these had me doing the technicolor yawn. Which is why I love coasters, not spin-&-pukes.

back on subject - I have never had a yack attack on a coaster. Although on a trip to BGT I did come close. I unfortunately drank a quart of OJ before begining the day. That combined with the heat and oh.. oh.. ooOOphhh. Good thing I had not ridden anything yet!
However, I will have to try looking up next time on one of those tea cup rides though. :)
