Iowa's Occupational Safety and Health Administration fined the theme park $4,500 this month in the death of Steve Booher. The 68-year-old man reportedly was unloading guests when the conveyor under the boats moved and he fell into the ride, sustaining several injuries, dying in the hospital several days later.
